In November 1999 the Institute of Medicine of the National Academy of Sciences reported 98,000 patients die every year in the United States due to preventable medical mistakes. Root Cause Analyst software is an automated, complex logic structure used to identify the causes of adverse outcomes, with the goal of preventing the recurrence of such events. It is the first automated product designed specifically to guide health care professionals through the performance of what risk managers and quality assurance professionals call a root cause analysis.
